Before Your Appointment
The Center for Spinal Disorders is dedicated to providing you with the most effective and efficient care possible. In order to accomplish this, we ask that you bring the following documents and information to your appointment:
1. Referral. If your insurance requires pre-authorization and/or a referral, please bring this with you to your appointment. Be prepared to provide complete and accurate information regarding your referring provider so that we can send correspondence once your appointment is completed. Without a referral, your appointment may need to be rescheduled.
2. All X-Ray, MRI, and CT films. These films will be used for all surgical decision-making and planning, therefore the report alone is not sufficient. Original radiology films or disc images are accepted.
3. Insurance Card. Bring your insurance card and any co-payment required by your insurance. (If you do not present with your co-payment you will either be rescheduled or have a $10.00 administrative fee assessed to your account for costs involved to bill you for your outstanding co-payment)
4. Medications List. We will need a complete and accurate list of your current medications (include vitamins, supplements, and herbs you regularly take).
5. Questions. Make a list of questions you have about your medical condition so the physician can address these concerns with you.
